• No deductibles, waiting periods or restrictions due to pre-existing conditions.
• Mail order services are not available through the prescription drug program.
• Fidelity Security Life Insurance Company (FSL) is the underwriter of the prescription drug program.
• Catalyst Rx is the pharmacy benefit manager.
